How to Add SOAP Notes, Patient Chart, Chart Notes, Record Keeping

We love our customizable SOAP Notes! If you have not already, check out our Article on setting those Macros up HERE!

Now, how do we add SOAP Notes to a patient record, you ask? Head on over to the Practice Manager and find the appointment under Today's Appointments. Find the appointment options (3 dashed lines) and select Outcome.

The first thing you will want to do is mark the patient In Treatment, which will move the patient out of the Checked In Column. If the patient has a previously Completed Appointment, SALT has the ability pull SOAP Notes, Diagnosis Codes, spinal adjustments, Line Items, and/or Treatment Information!

The Outcome screen will always default to the Overview of the patient's SOAP Notes. To input Notes, navigate through each SOAP tab (Subjective, Objective, Assessment, and Plan). If you have your own established set of Macros assigned to a Methodology Type, you will want to select that Methodology Type right above the Interactive Spine.

To use the Interactive Spine, simply select the desired vertebra. Selecting it a second time will increase the severity. Selecting the same vertebra will deselect the vertebra entirely. These selections will populate under the Assessment tab, and will dynamically update with each selection:

Diagnosis Codes can be added in two places: the Diagnosis tab or the Line Items tab, located right above the Interactive Spine:

Which one do you use? If you are creating a HCFA Form, we recommend using the Line Items tab. To learn more about the Line Item tab, you may CLICK HERE! If you only need Diagnosis codes for HSAs, HRAs, or Personal Injury Cases, the Diagnosis tab should work!
