Making a new form

Digital forms can help you save time, paper, and money in the office! It also helps your patients get in sooner for their visit without the hassle of having to rush through getting their intake done during their appointment.

With our AI Form Generator you can make forms just within minutes by following the directions below!


  1. Log into your Aloha Account

  2. Next, you will hover over Forms and click Show Forms.

  3. Select Import in the Top Right of the screen (only upload 1 PDF at a time)

  4. Click the Choose File option if you have the PDF on your computer or the Add URL Here if the form is a PDF placed on a website.

  5. Once the Form is submitted, the Edit Form page will open.

  6. Name your form, If you have an Aloha Hosted Site this is the name patients will see, so make sure to use a patient-friendly description and describe what the form is about, for example, “New Patient Intake Form.”

  7. Toggle on Save PDF; If enabled, a PDF will be automatically be saved upon submission of the form. The PDF will always show exactly what was submitted.

  8. Toggle on Send Email Notification; Send a custom notification to YOURSELF, to let you know that someone has completed a form.

  9. You will see a message that says “Your template is still being generated. Please check back soon!”

  10. Before exiting Click Save Form next to Generation Status.


11. Once this is done your form will have a Pending Status

12. once your forms is completed is will move to Needs Approval

13. You will select the form and make sure each page looks correct and hit "Approve Page" at the bottom of the screen for each form

14. If there is an image on your form PDF it will need to be uploaded into Aloha before hand and added to the form

15. After, you will then select Save Form

Once you have created the forms you want patients to use, you can upload them so patients can access them online. That way, you can send them to patients to fill out before they get to your office, which can speed up the patient check-in process and eliminate the need for updating paper forms in the reception area. Because forms can be uploaded to your practice website or sent to patients via email or text message, your patients have an easy way to fill out forms before they even arrive at your front desk.
