β Part 1: How to Get Your Form's Embed Code
Navigate to Your Forms. In Aloha, go to FORMS > Show Forms.
Select Your Form. Click on the name of the form you want to add to your site from the list.
Get the Code. On the form's page, you have two options for getting the code:
β Part 2: Best Practices for Installing the Code on Your Website
Pasting the code correctly is crucial for the form to work properly for your patients, especially on mobile devices. Here are our recommendations.
Use a Dedicated Page: For the best user experience, your form should be on its own blank page on your website (e.g., yourwebsite.com/forms/new-patient-paperwork). We do not recommend placing the form inside a scroll box or on a page crowded with other content, as this can make it harder for people to fill it out on their mobile devices.
Use an HTML Block: When editing your webpage, do not paste the code into a standard text box. Look for an HTML block, Code block, or Embed widget in your website builder. This tells your website to treat the text as functional code. The code should generally be placed within the <body> section of your page's HTML.
Special Note for WordPress: If your website uses WordPress, you may need a specific type of code. Go back to the form page in Aloha and look for an option to copy the Inline Frame code if the regular Embed Code doesn't function as intended. This version is often more compatible with WordPress sites.
