Let's say you uncover a new scenario after a PHA is completed - from an incident in the industry or a learning within your organization. Or through day-to-day operations, you find that a certain safeguard works better at mitigating a given scenario than what's documented in the PHA today. Or you're diving into your PHA data and notice some quality inconsistencies.

Typically, you would document these findings "somewhere" and then feed it to your team at the next PHA cycle a few years later. With LivePHA, you don't have to wait! You can capture these changes to your PHA data in real-time and feed updated analytics and insights back to your teams!

You can add or delete Nodes, Deviations, Causes, Consequences, Safeguards or Recommendation. You can even update severity or likelihood levels of Causes/Consequences. And much more! Reach out to your Risk Alive Customer Success team if you're wondering how you can update your PHA data with a specific finding at your site.

LIVE Analytics Updates

LivePHA has built-in validation checks that help ensure quality and consistency of PHA documentation in its original state and as it is updated based on findings at site. These validation checks are based on industry best practices for PHA documentation and Risk Alive learnings on data quality that yields the most valuable safety and operational insights.

Validation warnings and errors can be viewed in the top right-hand corner of the page (see Figure 2.)

Fig 2. Validation Warning Icon

The warnings are grouped into themes to help you navigate and resolve them efficiently. Clicking on a validation warning will take you to the exact location within the PHA data where the inconsistency is noted (indicated by a red exclamation mark). You can edit these within LivePHA to resolve these as you go. The Validation warnings and errors will update live as you resolve inconsistencies within the data.

If you have the appropriate automation tools turned on, you will have the option to automatically resolve these by clicking on the "Resolve" button under the corresponding group of warnings. If you would like to check the current available tools, you can do so under Settings.

The automation tool should take care of most of the warnings, but sometimes some warnings may require some extra attention. If the tool is unable to resolve the warning, the window in Figure 4 will pop up and provide extra details and next steps.

Once all the data inconsistencies have been resolved, you can now refresh your analytics reports with all of the changes you just made by clicking on the "Submit to Risk Alive" button! In this way, your site teams can have the most up-to-date information and insights to make risk-based decisions.

If you need to "bulk edit" cause, safeguard or recommendation description or category across the entire PHA, you can do so from the respective page accessible on the left side banner.
