Creating a care plan in SKED allows you to set clear visit recommendations for each practice member, helping them stay consistent with their care. In just a few steps, you can define start dates, set visit frequencies, and create a customized plan that displays directly in the SKED app. This ensures both your team and your clients stay aligned and on track throughout their care journey.
Begin in the Frequency tab of your client’s account and click Add.
Next, choose the Care Plan Start Date.
We recommend starting the care plan at the beginning of the week for a cleaner look in the app.
Next, set the patient’s Frequency.
You can choose from the preset options or create a custom frequency.
To add additional frequencies, click Add again.
📅 Example Setup
In this example, the client’s recommendations are set as follows:
Two times a week beginning the week of May 1st
One time a week beginning June 19th
Every three weeks beginning August 7th
If the care plan will be ending, you can add a frequency of zero beginning the week after the care plan is complete.
💾 Saving and Managing Frequencies
Once you’ve finished adding the client’s recommendations, click Save.
If you need to remove a frequency at any point, click Delete.
Adding frequency in the client’s SKED account will provide a visual for them in the app and help them stay on track with their recommendations.
