Report a Locate Request Concern

If something looks off on a locate ticket, such as incorrect mapping, contact details, or ticket content, the Locate Request Concern Form allows you to report the issue directly to our Stakeholder Support Department.


Once submitted, your concern will be reviewed and shared with the appropriate teams for investigation and resolution. Common reasons to use this form include:

  • The excavation polygon is incorrect or doesn’t match the described location

  • Contact information on the ticket is wrong

  • Streets or directions on the ticket are inaccurate or incomplete

  • GPS coordinates or driving directions lead to the wrong location

  • You didn’t receive a ticket in an area where you have registered assets

  • You received a ticket in an area where you have no assets registered

Providing the ticket number, a brief description of the issue, and any supporting details helps our team resolve the matter quickly and accurately.
