How to raise an incident request

What is a request

A request is the creation of a submission that allows you to send a form to the responsible team for them to take action. You will be able to follow the request and action through its progress and leave comments where applicable.


How to raise an incident request

  1. Open the 9-dot waffle menu in the top left corner in the monitor portal and select Incident Request.

  2. Click on the +Add button to raise an incident request.

  3. Select the appropriate request form and location, and then click Add.

  4. Once the form is completed, a green submit button will appear at the bottom. You can also add comments and upload media if needed.

  5. When the request is submitted, it will show in the Submitted section.
