Sending Forms From An Appointment

Learn how to manually send forms from a patients appointment through Aesthetic Nurse Software.

This article will guide you through how to send manual forms to a patient through ANS. By sending forms to your patients before they attend their appointments it means you as the practitioner are able to gain their medical history saving you time in your appointments.

Step 1 :

From the main menu, head to the diary area.

Step 2 :

Find the appointment you wish to send the forms for by heading to the date and then clicking on the appointment.

Step 3 :

Once you click on the appointment, you will be taken to an appointment summary where you can choose to click on send forms.

Step 4 :

From here you will be able to choose the questionnaire, consent and aftercare you would like to send to the patient.

The list of forms in this section is presented from the settings area, if there are any forms missing from this area they may be in your non active forms settings section. Alternatively, if you have a form which you would like adding to your software send this form over via email and we can get this added for you.

Step 5 :

Once happy with the forms selected, hit send.

This will then send the forms to the patient via email for them to complete at home and send them back to the appointment within ANS.


Step 6 :

If you would then like to view the forms sent to your patients. Again, click on the appointment in the diary and then the view sent forms button.

Any documents which have been completed by the patient will show as answered with any outstanding documents showing as sent.

Any completed forms can then be viewed in the clinical pathway of the appointment.
