How to View Incomplete Assessments on Carelever

Where you can find outstanding Carelever Forms and incomplete appointments on the Carelever Affiliate Portal

To find incomplete assessments on the Carelever Affiliate Portal there is two locations on the portal they could be situated:

  1. Today tab, OR

  2. Overdue tab

Depending on the date of the appointment of the incomplete assessment you will navigate to either the 'Today' tab or the 'Overdue' tab. Click on the 'Today' tab if the appointment is scheduled for today, or the 'Overdue' tab if the appointment is in the past.

Locating the Incomplete Referral/Assessment

Within the 'Today' or 'Overdue' tab, you'll see a list the list of referrals and appointments. Click on the name of the applicant, to open their referral.

The assessment for the applicant will have Carelever forms that need to be completed by your clinic. If these forms still display a 'Fill Up' link this means the assessment is incomplete and yet to be submitted.

Completing the Forms

Click on the 'Fill Up' link to open the Carelever Form. Review the Carelever forms and complete each section. Once a section has been completed in full, a green tick will appear on the form marking that section as complete.

You will be unable to submit the form if all sections do not have a green tick. Submit the forms once all required information is filled out and a green tick has been displayed on each section of the form.

Additional Assistance

If you experience any of the following:

  • The completed Carelever Form has been submitted but the referral is still sitting in your portal as overdue.

  • Unsure if you can complete one of the components.

  • Carelever Forms and paperwork (PDF) are unavailable to complete the assessment
