The Escalations & Actions report helps you identify the most common and most serious issues being reported by frontline workers across your organization.

What This Report Is Used For

Use the Escalations & Actions report to:

  • Identify top hazards and recurring issues

  • Understand where escalations are happening most often

  • Track how issues are being resolved over time

  • Prioritize training, controls, and corrective actions

It rolls up data across all sites and groups.

How Data Is Aggregated

This report relies heavily on workflow question tags.

Question tags allow ANVL to:

  • Group similar questions across different workflows

  • Aggregate results even if questions appear in multiple templates

  • Show trends regardless of where the data was captured

Example:

If multiple JSAs ask about PPE, tagging those questions allows PPE issues to be reported together.

Escalation Types Explained

The report breaks data down by escalation type:

Interventions – Elevated risk or warning conditions

  • Flags / Concerns – Issues explicitly raised by frontline workers

  • Stops – Serious hazards that prevent workflow continuation

You can filter the report to focus on one or more escalation types.

Key Sections of the Report

Top Question Tag Findings (Pareto View)

Shows the most common issues across all workflows, such as:

  • Training gaps

  • PPE issues

  • Equipment concerns

You can:

  • Sort by frequency

  • Click into findings

  • Drill down to specific workflows, users, and sites

Escalation Resolution Status

Displays how escalations are being managed:

  • Open

  • In Review

  • Closed

This helps track whether issues are being addressed and closed out.

Escalations Over Time

Shows how escalations trend across weeks or months.

You can:

  • See increases or decreases

  • Break trends down by site, role, or employee

  • Compare patterns across time periods

Raw Escalation Data

Provides a table-style view of all escalations in the selected timeframe.

This data can be:

  • Exported to Excel

  • Used for deeper analysis or audits
