What This Report Is Used For
Use the Escalations & Actions report to:
Identify top hazards and recurring issues
Understand where escalations are happening most often
Track how issues are being resolved over time
Prioritize training, controls, and corrective actions
It rolls up data across all sites and groups.
How Data Is Aggregated
This report relies heavily on workflow question tags.
Question tags allow ANVL to:
Group similar questions across different workflows
Aggregate results even if questions appear in multiple templates
Show trends regardless of where the data was captured
Example:
If multiple JSAs ask about PPE, tagging those questions allows PPE issues to be reported together.
Escalation Types Explained
The report breaks data down by escalation type:
Interventions – Elevated risk or warning conditions
