Create customized forms

This article will help you create and send custom forms to your members.

1st step: In the Bsport dashboard, click on "marketing" then "forms" in the drop-down menu on the left.

2nd step: Create your Form

Click on "Add a form" at the bottom right. Enter the name of the form then click on your form to configure it.

Step 3: Configure your form

Click on your form, then on the "Configure" button in blue.

Then click on the new element button to add lines to your form.

You can then add different lines to your form such as the title, multiple answers, simple answers, checkboxes, etc ... I will explain later how to do this.

Attention: Don't forget to save what you have done by clicking on save.

An example of your form is then available on the right.

4th step: Detail of the options

For the title, the paragraph, the short answers and the long answers, you just have to click on confirm in order to add the line and then to enter the title to write, the question to ask, etc...

Note: When you add a note in the short and long answers of the form, the answer will automatically be added to the member's profile in the notes section.

For the multiple choice, check boxes and drop down lists you must first add an option by clicking on "add an option". Once the option is created, you can add a tag to this option by clicking on the blue icon as shown below. You can then add a category and a tag to your option.

Finally, the signatures and files to be deposited, simply add the element by clicking on confirm.

Note: When a member uploads a document on the form, the document will be added automatically to the "Documents" section on the member profil page.
