Activity Forms can be attached to Activities and sent to Customers, Instructors, or Admins. We'll do this in a later step, but for now, you'll create Activity Forms.

Select the rocket ship icon at the top right-hand of the screen and select "My Company." Under the Forms tab on the top banner of the screen, select "Activity Forms." Select the large plus button.

You'll be prompted to create a title for the form from here.

Build this form by selecting the plus + icon. A design tool kit will pop up on the right. Add in a label and then decide how you want to collect this information by selecting an element!

After completion, select the Add button at the bottom of the screen. Continue making as many Activity Forms as you'd like!
