Simply drag and drop the Annotate Image field onto your form to get started.
Step-by-Step Guide:
To add the Annotated Image field, you have two options. You can either drag and drop it from the Fields menu or click the "Add Item" button and select it from the list of options.
When adding this field, be sure to fill out the Label and Description fields with detailed instructions on how you want your respondents to interact with the image you provide. For example, you can ask them to highlight or circle specific areas.
Next, upload the image you want your respondents to interact with.

If you want to make the field response mandatory, select Input Required.
You can capture the date and time stamp with each response by selecting Show Time Entered.
To capture the user details of the respondent, select Show User Last Edited.
To view the field responses at a glance, configure the Summary Page Settings.
• You can choose to filter responses quickly by selecting Filterable,
• View responses directly from the list by selecting Display in List,
• Edit responses in the display view by selecting Edit in List.
Select Save & Preview to see how the Annotated Image appears on your form
Complete designing your form and click Publish to start using it.
