If you need lab work, your medical provider will write a lab requisition.
Steps:
Get your lab requisition.
Make sure the information is accurate on it. Your name, date of birth, and health care number (if you have a health card number) must be as written on your government ID.
Bring the lab requisition to a lab of your choice. If you need help finding a lab near you, you can reach out to care@joinlivewell.ca for some assistance. Our Care Team will let you know about your options.
Where to find your lab requisition:
Go to your User Dashboard
You will have the ability to download your lab requisition
You will have the ability to send the lab requisition to your email
