How to Print a Return Form?
You can get a pre-filled Lifeway return form online by logging into your order history at Lifeway Order History and clicking "Return Items" next to your eligible order.
How to Get and Use the Return Form
Online: Go to your Lifeway Order History, find the order, select View, and click Return Items to download a pre-filled PDF return form.
Print and Pack: Print the PDF form and place it inside the box with the item(s) you are returning.
Alternative (Packing Slip): If you cannot use the online portal, you can use the return instructions and form details listed on the bottom left-hand corner of your original packing slip.
Key Lifeway Return Rules
Timeframe: Items must be returned within 90 days of delivery.
Condition: Items must be unused, unopened, in salable condition, and listed as in stock on the website.
Exceptions: Imprinted or engraved items are generally non-returnable unless defective.
Shipping Cost: Lifeway covers return shipping only if a delivery is refused; otherwise, you pay the return shipping costs.
