Build a form that collects applications, creates applicant records, and starts your enrollment workflows.

What an enrollment form does
An Enrollment Form asks respondents to sign in or create an account, then creates an applicant record. From there it can drive program selection, approvals, reminders, invoices, and agreement (signature) workflows. Student name and email address are collected automatically — you build everything else.
Steps
1. Open the form builder. In the left menu, go to Enrollment › Forms. Open the Forms tab and click New Form (top-right).
2. Choose how to start. In the Create Form dialog, pick Starting from Scratch to build every field yourself, or Starting from a Template to begin from a pre-built form. Click Submit.
3. Fill in the General Details. Give the form a Form Name and a URL identifier (the slug used in the form's public link). Set Type to Enrollment Form, add an optional Description, and choose any Default labels (auto-applied to the applicant contact) or a default Program. Leave Program blank if applicants should choose from several — you'll add a Program Selection field later.
4. Set the form's behavior. Configure how the form runs after someone applies: Approvals (turn on Auto Approval to approve automatically, or leave it off to review each one), Reminders (nudge applicants who haven't finished), Completion Messages (customize what people see after they submit), and CRM Mappings (optionally sync form events to Contact custom fields).
5. Build your pages and fields. Under Pages, give the page a Page Title, then click Add Field and choose a field Type for each question. Use Add Page to split a long form into multiple steps, and the arrows to reorder. Student name and email are already included, so start with the extra information you need.
6. Publish and create. Set Status to Published when the form is ready to accept applications (leave it on Draft to keep working). Click Create to save. Use Create & create another if you're setting up several forms in a row.
Field types you can add
Prebuilt applicant fields: student's first name, last name, email, phone number, birthdate, address; emergency contact details & address; program selection; desired start date; mandatory agreement; digital signature; document download (admin uploads); document upload (from student).
Generic inputs: short text, long text, date, number, checkbox, select from list — plus static text and static image for instructions.
Tips
Keep the URL identifier short and readable — it becomes the public link. Split long forms across multiple Pages so applicants aren't overwhelmed. Add a Program selection field (and leave the form-level Program blank) when one form serves several programs. Add a Digital Signature field to capture agreements right in the form.
