Overview
As part of the ISLHD Graduate Nurse Program, you may be assigned forms to review and complete on behalf of your graduates through Med App. This article explains how to access, complete, and manage these forms.
Step 1 – Receiving Your Form Notification
You'll receive an email (or SMS) notification letting you know a form has been assigned to you. The email will come from the Med App / ISLHD Graduate Program and will include a direct link to the form.
What to look for:
Subject line referencing the grad's name or form type (e.g. "Learning Plan")
A clearly labelled button or link: "Complete Form"
Click the link to open the form in your browser.
Step 2 – Reviewing Your Graduate's Response
Once the form opens, you will be prompted to check your details and any notification preferences for future forms.
Then you'll be able to see the graduate's completed section at the top of the form.
You can read through their responses before proceeding to your section.
Step 3 – Completing Your Section
Click continue to find your section of the form. This is where you add your own feedback, ratings, or comments depending on the form.
Fill in all required fields. Required fields are typically marked with an asterisk (*).
Step 4 – Saving as a Draft
If you need to step away before finishing, you can save your progress as a draft — your responses won't be lost.
Look for the "Save as Draft" button, in the bottom right of the form.
To resume, simply click the link in your original email/sms notification again — it will take you back to your in-progress form.
Step 5 – Submitting the Form
Once you've reviewed the grad's responses and completed your section, click "Submit" to finalise the form.
You'll see a confirmation message once the form has been successfully submitted.
Keeping Track of Your Forms — Med App
If you'd like to view a history of forms you've completed or have pending, download the Med App on your mobile device.
Download the Med App:
Once logged in with your ISLHD credentials, navigate to the 'more' tab then My Forms to see:
Forms awaiting your completion
Previously submitted forms
Draft forms in progress
