Med App is the platform used to manage Workplace Based Assessments (WBAs) digitally at a number of Australian WBA locations. As an assessor, you will use Med App to receive assessment requests and submit your feedback — no paper forms required.
New to Med App? You should have received an invitation email/SMS from your Hospital. If you haven't received it, contact your WBA coordinator before proceeding.
Getting started & logging in
You can access Med App from mobile or tablet, or get Form notifications via your registered email.
First-time setup
Open the invitation email from Med App and tap the registration link.
Download the Med App from the App Store or Google Play.
Click 'register now'
Follow the prompts to confirm your name and set a password.
Your hospital location will appear automatically under "My Locations".
Returning users
Download or sign in to the Med App mobile/tablet app. You can update form notification preferences so that you can receive forms via email rather than the mobile app if preferred.
Receiving an assessment request
The WBA Co-ordinator assigned assessments to the candidates. The candidate will then schedule a time/date with you to perform the assessment. Once a candidate has assigned you an assessment, you will be notified via SMS, push notification or email (as chosen in your Med App settings).
Open Med App and go to More > My Forms to see your pending assessments.

Forms awaiting your input will show as 'to be actioned'.
Your assessments are pre-assigned. You don't need to search for the candidate — the form will already show their name and the assessment type when you open it.
Completing the assessment form
Review the candidate's details in their completed section (e.g. name, AMC number). This section is read-only for you.
Complete your assessment section. This typically includes:
Performance ratings across clinical domains
A global rating or entrustability level
Free-text feedback — strengths and areas for development
NOTE: The clinical domain (e.g. Adult Surgery) will be pre-selected so you should not need to update this.
Sign your section using the on-screen signature field.
Tap Submit to complete the assessment. The candidate will be notified automatically.
Completing on a computer? Tap the desktop icon in the top-right corner of the form, then open a browser on any computer and go to web.med.app/form. Enter the unique code shown on your phone to continue.
WBA form types
WBA programs use some of the following assessment types. The form you receive in Med App will be labelled with the assessment type.

Mini-CEX | Direct observation of a clinical encounter — history taking, examination, clinical reasoning, communication. |
CBD | Structured discussion of a clinical case the trainee has managed — tests clinical reasoning and decision-making. |
DOPS | Direct observation of a specific procedural skill performed by the trainee. |
MSF | Anonymous 360° feedback on professional behaviours and teamwork. Completed independently by each rater. |
ITA | Overall assessment of the trainee's performance across a rotation or term. Completed at end of term. |
After you submit
The candidate is automatically notified that their assessment has been completed.
The WBA Co-ordinator can see your submission in the admin dashboard — no follow-up is required from you.
Your feedback becomes part of the trainee's WBA record.
For MSF forms, your response is anonymised before the candidate receives their generalised feedback report as per each WBA program's procedures.
Need to correct a submitted form? Once submitted, forms are locked to maintain record integrity. Contact your WBA Co-ordinator — they can unlock the form for amendment via the admin dashboard.
Common questions
Q: I didn't receive an invitation email — what do I do?
A: Contact your WBA Co-ordinator. They can resend your invitation from the Med App admin dashboard. Also check your junk/spam folder and search for noreply@med.app.
Q: Can I complete the form on my computer instead of my phone?
A: Yes. In the top-right corner of any open form, tap the desktop icon. You'll be shown a unique code — open a browser on any computer, go to web.med.app/form, and enter the code to continue on the larger screen.
Q: Can I fill out a form if I'm offline?
A: No — you need an internet connection to submit a form. If your connection is unreliable, save a draft while connected so your progress is not lost.
Q: I submitted feedback but made an error — can I edit it?
A: If you just submitted it, you can retract it and make changes then resubmit. If the candidate has completed their sign-off it will be locked. Contact your WBA Co-ordinator with the trainee's name and form type — they can help amend it.
Q: Does the candidate see my name on MSF feedback?
A: No. MSF responses are anonymised. The candidate receives a collated feedback report only — individual rater identities are not disclosed.
Q: I'm assigned to multiple trainees — how do I switch between them?
A: All forms assigned to you appear in More > My Forms. Each row shows the trainee's name, form type, and current status. Tap any row to open that form.
