When setting up a printable registration form, there may be fields on the paper form with checkboxes to select. However, when populating the form with data there is no way to check a box as the system can only insert some saved data about the player. A common example might be a Gender question (with checkbox options of 'male' and 'female) or a Relation to Player question (with checkbox options of 'father', 'mother', or 'guardian').
We recommend modifying the form image to change the question from a check box to a blank field. To do this, open the form image in an image editor (pixlr.com/editor is a free online tool) and add an all-white rectangle over the check box options for the question. This will effectively remove the checkbox options from the form. You can then save the image and upload it to the printable form for use in the system. The data field containing the question response for the previous check box question can now go directly on top of the area that was whited out!
Attached to this solution is the US Club Soccer Medical Release Form, which has been updated for use with our Registration system.
