When members join your club, they fill out a registration form. Build Your Reg Form is where you set that form up.
What's on the form by default
Every registration form starts with an Accept Club Terms & Conditions card. This is mandatory and cannot be removed. PLAI pre-loads a template Terms & Conditions document that you can edit.
To replace it, tap the edit icon on the Terms & Conditions card, then upload or link your own document from the bottom sheet that opens.
Adding a question
Tap Begin on the Build Your Reg Form card.
Tap + to open the question type selection screen.
Choose one of 6 types: Short Answer, Paragraph, Single Select, Multi Select, File Upload, or Date.
Enter your question text.
Toggle Required on or off.
For Single Select or Multi Select, also enter each option value.
Repeat for as many questions as you need. You can edit or delete any existing question by tapping its row, and reorder questions by dragging a card up or down.
Note: A form with zero additional questions, just the Terms & Conditions card, is a completely valid, completable state.
Previewing and finishing
Tap Preview to see the form exactly as a member would.
Tap Complete Setup (gold button) once you're happy with it.
You are returned to the checklist, and Build Your Reg Form shows a green tick with a confirming toast.
Editing after completion
Tap the edit icon on the completed card to reopen the form editor with all existing questions and Terms & Conditions settings pre-loaded.
Important: Changes you make after completion apply to future registrations only. They do not retroactively change responses members have already submitted.
FAQ
Do I need to add extra questions at all?
No. The Terms & Conditions card alone is a valid form. Popular additions if you do want more are emergency contact details, medical notes, or shirt size.
What if I try to save a Single Select or Multi Select question with no options?
You will see: "Please add at least one option." The question cannot be saved until at least one option is entered.
