Forms let your club collect structured information, signatures, and payments from members in one place, from surveys to registration paperwork to trip sign-ups.
What a form can include
When building a form, admins can add any mix of 7 question types: short answer, paragraph, multi-select, single select, file upload, date, and signature. A signature question can have a document attached (like a set of terms and conditions) that members must scroll through before they can sign.
Note: A signature question always requires the member to type their full name to sign. When a parent signs on behalf of a child, the record shows the parent's name against the child's response.
Who can create a form?
Only club admins can create and publish forms. Admins choose which groups can respond, and can optionally set an expiry date and collect a fee.
Important: Once a form receives its first response, its questions and fee settings lock. This keeps everyone's answers comparable, so review a form carefully before publishing.
Responding to a form
Members find open forms in the Forms tab. If your account has linked family members, you will be asked who is responding before you start, and each person's answers are completed and submitted separately, even if you are filling them out in one sitting.
If a form has a fee, you pay for everyone you have just responded for in a single combined checkout once all their answers are complete.
Managing responses
Admins can search, filter, and review individual responses from the Manage tab, including any uploaded files, signed documents, and payment details. A response can be refunded or deleted without affecting anyone else's submission, and members are able to resubmit if a response is deleted while the form is still open.
FAQ
Can I edit a form after it has been published?
Yes, but only until the first response comes in. After that, its questions and fee settings are locked to keep every response consistent.
What happens to payments if I delete a response?
Deleting a response does not automatically refund the payment. Refunds and deletions are two separate, deliberate actions an admin has to take.
Do I lose my progress if I close the app mid-form?
Your answers are cached locally where possible, so you can usually pick up where you left off when you reopen the form.
