How to View Your Submitted Form Responses and Receipts

How to find your past form responses and payment receipts, including forms you submitted on behalf of a linked family member, and how to download a receipt or dispute a payment.

Once you've submitted a form, you can go back at any time to review your answers or check what you paid.


Viewing your completed forms

Expand the N Completed section in your Forms feed, or open a specific completed form, and tap View My Responses. You'll see one row per respondent, yourself and any linked family members you submitted for, each marked "Form Submitted" with a View Response link.


Viewing a response

Tap View Response to see the respondent's name, submission date, the form and fee summary, and every question with its submitted answer, including uploaded files and signature attestation ("Signed by [Name] • [date]").

Note: If the response was later deleted by an admin, you'll see "This response is no longer available" instead.


Viewing your payment receipt

If the form had a fee, tap the fee line on the response to open the payment detail card. It shows:

  • Amount paid, with a Paid status badge

  • Fee breakdown (Form Fee and Checkout Fee)

  • Source, Payment Method, and Payment ID

  • An activity timeline (payment started, then succeeded, with timestamps)

From here you can:

  • Tap Download Receipt to save or share a copy for your records.

  • Tap Dispute Payment if something's wrong, this routes you into a separate dispute flow with your club or the payment provider.

Note: Forms with no fee don't show a payment card at all.


FAQ

Why can't I find a response I submitted before?

If an admin has deleted that response, it will no longer appear or will show "This response is no longer available." You won't be notified automatically when this happens.

What happens when I dispute a payment?

You're routed to a separate support flow to raise the issue with your club or the payment provider. Disputing a payment doesn't change your submitted answers or the form itself.
