Enrollsy has an easy way to do this! Just follow these steps:
Step 1 - Program Selection
Go to My Company > Forms > Enroll Form. Scroll down to Program Selection. Click the plus sign and add your title.
Step 2 - Add New Field
After adding your title, go down and click the plus sign to add a new field:
Step 3 - Enter Label
Next, type in your label (see example below) and click Date:

Step 4 - Add Form to Program(s)
Next, you will need to add the Program Form to the programs it pertains to. You can do this for multiple Programs.
If you need to add it to just one program, go to your Program Setup Form (My Company > Programs > Pencil icon on the right side).
Scroll down to the Enroll Form section (see below) and click on the down arrow under Additional Program Fields.
Check your newly created Start Date Program Form and click save.
