How to create a survey template in Med App

Create survey templates in Med App and send surveys with ease. Save time, get feedback, and improve clinician support.

In this article, we'll show you how to create a reusable survey template to reduce your time and effort.

Creating a survey template

To create a survey that can be distributed to clinicians, you must first create a 'survey template'.

  1. Open the 'Surveys' feature on the left navigation bar of the dashboard.

  2. From the survey's homepage click on the 'Create Survey Template' button.

  3. Enter your template name & description e.g. 'Education survey'. This will help other administrators identify what the template should be used for.

Configuring your Survey

Here you construct the survey questions and answer types.

  1. Write a short survey introduction for clinicians to describe the purpose of the survey.

  2. Add questions to your survey, selecting the different survey question & answer types.

  3. Save your survey.

Help! I can't save my survey.

Make sure all questions have text, otherwise, you won't be able to save the survey.

Reordering questions

Change the order of questions by clicking on the up/down arrow next to a question number. You can change it to before or after any question in the survey.

Preview & save your survey template

See how your survey will look to clinicians before you save your template

  1. To make changes, go back by clicking on the previous step in the workflow progression.

  2. Once you preview your survey template, you can save it and use it for future survey distributions.

Managing survey templates: Editing, Cloning and archiving

From the survey screen, you can now manage all of your existing survey templates and perform several useful functions: previewing, editing, cloning & archiving.

Next steps and distributing your survey template

Well done! You are now ready to distribute the survey to your clinicians.

Once you have saved your survey template you can use the template during your event creation process or send it directly to clinicians via a mobile push notification.
