Release of Information: How to Create a New Request

Add a medical records request to the Release of Information (ROI) Dashboard, either from the Dashboard or straight from a fax, upload, or print job in your Inbox.

What is an ROI request?

When you receive a request for medical records, whether in person, by phone, fax, or mail, you can add it to the Release of Information Dashboard. This keeps all your requests in one place so you can track them.

You can create a request two ways:

  • From the ROI Dashboard

  • From the Inbox, using a fax, upload, or print job

How do I create a request from the ROI Dashboard?

  1. Select ROI from the left-hand menu.

  2. Select Dashboard in the top right corner. The Dashboard usually opens by default when you select ROI.

  3. Start a new request. The request creation screen opens.

  4. Fill in the request details (see the table below).

  5. Select Create.

How do I add the patient?

If the patient is in your Updox Address Book: Start typing their name, then select them from the list.

If the patient isn’t in your Address Book: Create a one-time entry for this request.

Note: One-time entries are only used to label this request. They’re not added to your Updox Address Book and don’t sync with your EHR.

Tip: A one-time entry doesn’t have to be a patient. You can use any label you like, so you can use ROI for other kinds of requests besides medical records.

What should I put in “Requested By”?

Whatever helps you remember where the request came from. For example:

  • Simple: “Parents”

  • Detailed: The business name and the fax number to send the records to

How should I use the Due Date?

Set the date the request needs to be finished. Many requests don’t have a deadline, so you can also use the due date to set priority. Pick an earlier date for urgent requests.

The default due date is 30 days from when you create the request.

What is the Request Document?

It’s the paperwork you received from the requestor, such as a scanned letter or a PDF that came by email.

  • You can add only one request document per request.

  • It’s optional.

  • If you don’t have it yet, you can add it later.

What are Notes for?

Notes are for your team. For example, you can ask a specific person to look at the request, or mark how urgent it is.

Notes appear on the request details page. They are not included in any documentation you send out.

How do I create a request from the Inbox?

If a fax in your Inbox is asking for medical records, you can turn it into an ROI request directly:

  1. Open the fax, upload, or print job in your Inbox.

  2. Select Create ROI Request.

  3. The request creation screen opens, with that item already attached as the Request Document.

  4. Fill in the other fields.

  5. Select Create.

Your new request appears in the ROI Dashboard.

Note: Create ROI Request is available on faxes, uploads, and print jobs in your Inbox.

I don’t see the “Create ROI Request” button. Where is it?

It may be under More in your Inbox buttons.

To move it so it always shows, an admin can:

  1. In the Inbox, select Admin (top right), then Inbox Buttons.

  2. Make sure Create ROI Request is checked.

  3. Drag it into the order you want.
