How do I start a new form?
In the left-hand menu, click Forms.
Click Management in the top right.
Click Add New Form.
Choose a form template, or choose Blank Form to start from scratch.
If you chose Blank Form, click Create Form.
Step 1: Name your form and choose where it goes
Name your form
Give your form a name.
Patients see the form name when they open the form, so choose a name that makes sense to them.
Send completed forms to a queue (optional)
If you use queues, choose a queue under Submit To. Completed forms will go straight to that queue.
Tag completed forms automatically (optional)
If you use tags, choose a tag. Completed forms will be tagged automatically, so they're easy to filter in your Inbox.
Example: For an insurance update form, set Submit To to your Billing queue and add a Needs Reviewed tag.
Portal options
If your practice uses the Updox Patient Portal, you'll also see Active on Portal and Public on Portal. If you don't use the Patient Portal, these options won't appear.
