Managing forms
Can I combine several forms into one?
No. You can't copy, paste, or merge content between forms right now.
Tip: To send several forms together, create a packet instead.
Can I show or hide questions based on a patient's earlier answers?
No. Conditional questions, which show or hide depending on earlier answers, aren't available right now. This may be added in the future.
Why can't I use certain field types, like Signature or Image Upload?
Your form is probably set up as a portal form. There are two types of forms:
Form type | Where patients find it | How you set it up |
Portal form | On the Patient Portal, under the Forms tab | Active on Portal or Public on Portal is checked |
Web form | By a link (URL) you send or post | Portal boxes are unchecked |
When a form is set as a portal form, these field types are turned off:
Signature
Star Rating
Link
Image Upload
Tip: To use these field types and offer the form on your portal, leave the portal boxes unchecked. Then paste the form's link into the About Us section of your Patient Portal.
Where do I find a form's link?
In the left-hand menu, click Forms, then click Management.
Click the form.
Click the three dots (⋯).
Click Copy Form Link.
Note: Don't shorten form links with Bitly or TinyURL. Text messages with shortened links are blocked.
Sending and tracking forms
Why isn't a form I sent showing on the Forms Dashboard?
Forms completed from a copied link go to your Inbox, but they don't show on the Forms Dashboard. To track a form, send it with Send Forms on the dashboard.
How long do assignments stay on the Forms Dashboard?
30 days by default. You can change this in Forms > Settings.
How big can an uploaded image be?
The Image Upload field accepts images up to 10 MB.
