What Patients See When They Complete Forms

A walkthrough of what patients see after you send them Updox forms or a packet: opening the link, starting and moving between forms, tracking their progress, and submitting.

What happens after I send forms to a patient?

The patient gets a link to their forms by text or email, depending on how you sent them.

When they open the link, they can fill out and submit the forms online.

What does the patient see when they open the link?

The patient sees a landing page that lists every form assigned to them.

To start, the patient clicks Begin. This opens the first form in the list.

How does the patient move to the next form?

At the end of each form, the patient clicks Submit and Continue. This submits that form and opens the next one.

Can the patient see how many forms are left?

Yes. The patient's progress shows in the top left of the screen.

How does the patient finish?

On the last form, the patient clicks Submit and Finish.

The patient then sees a success message confirming their forms were submitted.

Where do the completed forms go?

Completed forms are delivered to your Inbox.

If you sent the forms from the Forms Dashboard, the status there changes to Completed.

Can patients fill out forms on their phone?

Yes. Patients can complete forms on a phone, tablet, or computer.
