Your accounts payable or vendor setup team can use this form to add Updox as a vendor.
Why would I need Updox’s W-9?
Many practices need a W-9 from their vendors to:
Set Updox up as a new vendor in their accounting system
Complete vendor onboarding forms
Pay invoices by ACH or check

Important: A Support PIN is required for all billing requests. To find it, select your initials in the lower-left corner of Updox, then select the eye icon. Your Support PIN changes after each use.
