Onboarding Checklist: Forms with Text

A setup checklist for Updox Forms: create users, build your forms, send them to patients by text, and manage completed forms.

Who is this checklist for?

This checklist is for the Admin setting up Updox Forms. Follow these steps in order to get your practice ready to send forms to patients.

Step 1: Create your users

  1. In the left-hand menu, click Inbox.

  2. Click Admin in the top right, then click Users.

  3. Click New and add each person who will send or review forms.

For full steps, see Creating and Managing Users.

Tip: Use each person’s email address as their username. It’s easy to remember, and it lets users reset their own password if they forget it.

Step 2: Learn how Forms work

Read these first to get familiar with Forms and the Forms Dashboard:

Step 3: Create your forms

Copy a form from the Form Library or build your own.

Step 4: Send forms to patients

You can send one form or a packet of several forms at once, by text or email.

Step 5: Manage completed forms

When a patient finishes a form, it comes back to Updox for your team to review.

Step 6: Set up text templates

Templates save time on the messages you send most, like asking a patient to complete forms before a visit.

  1. In the left-hand menu, click Inbox.

  2. Click Admin in the top right, then click Templates.

  3. Click New in the top right.

  4. Enter a Title and the message.

  5. Check Active, then click Save.

Note: For regular text (SMS) messages, a template must be 280 characters or fewer.

For more, see the Templates User Guide.
