Settings - Online Booking Forms

Set up the form your customers fill out when they request service from your website.

An Online Booking Form lets new and existing customers request service from you online — no phone call needed. Set up the form once, share the link, and customer requests flow right into Fieldwork.

How to Get to Online Booking Forms

In the upper-right corner of your screen, click the Settings gear icon.

Under Company Setup, click Online Booking Forms.

Click Create a New Online Booking Form (or choose an existing one to edit).


Name

This is the title of the form that you and your team will see in Fieldwork. Customers don't see this name — it's just for keeping your own forms organized.

Tip: Pick a name that describes the form's purpose, especially if you plan to create more than one. For example:

  • General Service Request

  • Quote Request – Residential

  • New Customer Signup


Description

This is a short note that appears next to the form's name in the Online Booking Forms List inside Fieldwork. It's another reminder for you and your team — customers don't see this either.

Examples:

  • "Linked from the homepage – for new customer requests."

  • "Used on the pest control landing page."

  • "For quote requests only — does not create a service job."

If you only have one form, you can leave this blank or write something simple like "Main booking form."


Branch

If your company has more than one branch (like different cities, service areas, or business divisions), this setting controls which branch any new customer from this form will be created under.

The default is Main Branch.

Why it matters:

  • The customer's branch decides which team sees their requests

  • It affects which branch's company info shows up on their invoices and emails

  • It controls which staff and pricing rules apply to the customer's account

Example: If you run a pest control business with a North office and a South office, you might create two booking forms — one for each — and set each form's branch to match the corresponding office. That way, requests automatically land with the right team.

If you only have one branch: Leave this set to Main Branch. You don't need to change anything.


Saving Your Form

Once you've filled in the Name, Description, and Branch, click the green Create button in the lower-left corner of your screen. Your form is now ready to be shared with customers.


Cancel Your Form

If you have filled out the Form but do not want it to be shared with customers, click the grey Cancel button at the lower-left corner of your screen
